Guiding You Through Every Step of the Care Journey
Navigating the healthcare system can be confusing — especially when your loved one’s needs keep changing. Between hospital discharge plans, home care eligibility, and community supports, families are often left feeling overwhelmed. That’s where Care Coordination & Advocacy makes all the difference.
At Daily Home Care, we provide care coordination and client advocacy services in the Niagara Region to help families understand their options and access the right support. We act as your partner — communicating with healthcare teams, retirement homes, and community organizations to ensure that care remains consistent, clear, and person-centered.
Our Care Manager helps families organize care plans, arrange services, and anticipate future needs — especially after hospital discharge or during times of transition. Whether it’s reviewing care goals, clarifying funding programs, or simply speaking on your behalf, we’re here to make sure your voice is heard and your loved one’s care truly aligns with their needs and values.
We will support you in:
-
Understanding options available through home care, government-funded programs, and private services
-
Navigating complex systems like hospitals, community services, insurance, or social services
-
Speaking up when something doesn’t feel right—or when care decisions need more clarity
You don’t have to face it alone. We’re on your side.

Coordinating care is not just about logistics – it's about advocacy, understanding, and human connection.
Why Advocacy Matters
Advocacy is essential in home care because it ensures that clients receive personalized, respectful, and high-quality support. Often, individuals and families face complex healthcare decisions and system barriers that can lead to confusion or unmet needs. A dedicated advocate helps navigate these challenges by amplifying the client’s voice, coordinating care, and safeguarding their rights and preferences. This proactive approach not only improves care outcomes but also fosters dignity, trust, and peace of mind for clients and their families during vulnerable times.
How We Support You
Our coordination and advocacy services are flexible and personalized. They can be provided as a standalone service or integrated into your loved one’s ongoing care. We help ensure that the care plan in place moves forward and leads to results
