What is FMHC? — Ontario's Self-Directed Home Care Option
Family-Managed Home Care (FMHC) is a program offered by Ontario Health atHome that provides families with more choice and flexibility in arranging care. Instead of care being scheduled by the system, families are given funding and can select their own care provider.
This means families can finally bridge the gap between what’s free but limited and what’s flexible but costly.
The Opportunity and The Challenge
FMHC can be life-changing. Families have flexibility to:

Arrange care around their own schedules and routines.

Access funded support instead of paying entirely out of pocket.
But with that flexibility comes new responsibility. Families are expected to oversee the funding and manage care delivery in line with Ontario Health atHome’s requirements. For many, this can feel overwhelming — on top of everything else they’re already doing.
Who Qualifies for FMHC?
Family-Managed Home Care isn’t for everyone — but for those who qualify, it can be life-changing.
Families may be eligible if:

An adult is living with an acquired brain injury

A child is being home-schooled

Or, when a patient’s situation falls under extraordinary circumstances
