What is FMHC? — Ontario's Self-Directed Home Care Option

Family-Managed Home Care (FMHC) is a program offered by Ontario Health atHome that gives families more flexibility in arranging care. Instead of services being scheduled through the system, eligible families receive funding and can choose their own care providers.

This allows families to access funded support while maintaining flexibility in how care is arranged at home. 

The Opportunity and the Challenge

FMHC can be life-changing for families who need more flexibility in arranging care. It allows families to:

  • choose caregivers who fit their loved one’s needs and values

  • arrange care around their own schedules and routines

  • access funded support instead of paying entirely out of pocket

With that flexibility comes responsibility. Families must manage funding and oversee care delivery in line with Ontario Health atHome’s requirements, which can feel overwhelming while supporting a loved one.

Who Qualifies for FMHC

Family-Managed Home Care is not available to everyone, but for those who qualify, it can provide meaningful flexibility in arranging care.

Families may be eligible when:

  • a child has complex medical needs

  • an adult is living with an acquired brain injury

  • a child is being home-schooled

  • or when a situation falls under extraordinary circumstances

Eligibility is determined by Ontario Health atHome. 

Our role is to help families understand their options and support care delivery once FMHC funding is in place.

Once FMHC funding is approved, families can choose how care is arranged and delivered. We support families by helping turn funding approval into dependable, day-to-day care.

Services We Provide Under FMHC

Under Family-Managed Home Care, families can choose the types of support that best meet their loved one’s needs. We provide a range of non-medical home care services that may be arranged through FMHC funding, including:

  • Personal Care 

  • Companion Care

  • Dementia Care

  • Respite Care

  • Palliative Support

  • Post-hospital Support

Services are arranged in collaboration with families to ensure care remains consistent, appropriate, and aligned with program expectations.

Getting Started with FMHC

Beginning care through Family-Managed Home Care typically involves a few steps coordinated with Ontario Health atHome.

1. Assessment and eligibility
Ontario Health atHome determines eligibility for FMHC through assessment and approval.

2. Funding authorization
Once approved, FMHC funding is authorized for eligible care services, which are coordinated and billed according to program requirements.

3. Care begins

We work with families to coordinate caregivers and begin services based on their needs and schedule.
