What is FMHC? — Ontario's Self-Directed Home Care Option
Family-Managed Home Care (FMHC) is a program offered by Ontario Health atHome that gives families more flexibility in arranging care. Instead of services being scheduled through the system, eligible families receive funding and can choose their own care providers.
This allows families to access funded support while maintaining flexibility in how care is arranged at home.
The Opportunity and the Challenge

Who Qualifies for FMHC
Family-Managed Home Care is not available to everyone, but for those who qualify, it can provide meaningful flexibility in arranging care.
Families may be eligible when:

a child has complex medical needs

an adult is living with an acquired brain injury

a child is being home-schooled
-
or when a situation falls under extraordinary circumstances
Eligibility is determined by Ontario Health atHome.

Getting Started with FMHC
Beginning care through Family-Managed Home Care typically involves a few steps coordinated with Ontario Health atHome.
1. Assessment and eligibility
Ontario Health atHome determines eligibility for FMHC through assessment and approval.
2. Funding authorization
Once approved, FMHC funding is authorized for eligible care services, which are coordinated and billed according to program requirements.
3. Care begins
We work with families to coordinate caregivers and begin services based on their needs and schedule.
